Born in London, England on January 29, 1924, he was the son of Louis and Jane Lazemnikoff.

Mr. Lewis was a veteran of World War II, serving in the Royal Air Force (R.A.F.).

He then became a self-employed cabinet-maker and antique-restorer.

Jack was survived by his wife, Irene (Kauer) Lewis.

The burial was private.
